On the first visit:

  5. Form 1050 Authorization for use and disclosure of medical information,date and signature.
  6. Copy of patient’s drivers license
  7. Insurance company name
  8. Claim Number
  9. Adjuster’s name
  10. Adjuster’s phone number
  11. Adjuster’s fax number
  12. Insurance claim mailing address
  13. Treatment plan,Dr. Choon Kia Yeo signature and fax to adjuster for approve
  14. Claim form CMS 1500; ICD CODE, CPT CODE.
  15. Keep each visit in excel form

Treatment plan:
  1. Call adjusters to get the necessary information to fill out the treatment plan if patient does not have all the information.
  2. Let Dr. Choon Kia Yeo M.D. signature. Fax to adjuster for approve.
  3. If faxing a Treatment Plan does not work, call the adjuster for their email address and email them the Treatment Plan.
Treat patient:
  1. Try to schedule Auto Injury patients when Massage Therapist comes in.
  2. Keep all patients charts organized by stapling claims information and visit sheet together. Do not staple the original Treatment Plan to anything.
  3. Make sure the total visit under approve plan.
Prepare claims:
  1. Prepare claims (includes Treatment Plan, Medical Notes, CMS-1500 forms, and envelope).
  2. Make a copy of Treatment Plan for claim, one copy each for acupuncture and massage claims (original copy of Treatment Plan stays in patient’s chart).
  3. Do Medical Notes for each visit in PracticeFusion website, print out each Medical Notes for claims.
  4. Fill out CMS-1500 on computer, print on CMS-1500 paper. Have Dr. Cai and massage therapist sign CMS-1500 forms. Once signed, make copies of CMS-1500 forms, keep the copies in patient’s chart. Write on the top page of copies date claim was mailed out.
  5. Staple each CMS-1500 form to it’s respective Medical Notes dates. Put all claim paperwork in big envelope. Write claim number on the bottom of the envelope below the claim address.
  6. Call adjusters to check on status of claims.
Payment received:
  1. When payments are received, go over payment information to make sure that payment is complete. If payment is not complete or $0, read notes to find out why (if not clear, call adjuster to find out why). Make the changes and resubmit the necessary information.
  2. Input check payments and electronic payments into the square app on the tablet.
  3. Stamp the back of all checks with the Aloha Injury Clinic stamp.
